Abstract
Background: Lateral ankle sprains are the most common acute sports injury, particularly in indoor court sports. Aim: To estimate prevalence and describe injury mechanisms among Danish elite and sub-elite handball, badminton, and basketball players, and to examine associations with athlete characteristics and kinesiophobia. Methods: A retrospective questionnaire was completed by 1,238 athletes (925 handball, 207 badminton, 106 basketball). Data included demographics, anthropometrics, weekly training hours, competition level, ankle sprain history and recency, injury mechanism (contact vs non-contact), perceived pain, and fear of reinjury. Analyses used logistic regression, chi-square tests, ANOVA, and Spearman correlations. Results: Overall, 73.8% reported a prior lateral ankle sprain and 28.7% had sustained one within the past 12 months. Past-year risk was higher in handball and basketball than in badminton, with no difference between handball and basketball. Badminton players sustained a larger proportion of non-contact sprains, while mechanism distribution did not differ between handball and basketball. Independent risk increases were associated with larger shoe size, higher competition level, and greater weekly training hours, whereas age showed an inverse association, indicating greater risk in younger athletes. Fear of reinjury showed weak negative correlations with both time since the most recent sprain and current ankle pain. Conclusion: Lateral ankle sprains are highly prevalent in Danish elite and sub-elite indoor athletes, with sport- and mechanism-specific patterns and exposure-related risk factors, informing targeted prevention and rehabilitation that also addresses kinesiophobia.
