Abstract

Colorectal cancer (CRC) is a serious global health issue, causing about 935,000 deaths each year. To address this, the Department of Surgery at Zealand University Hospital, Køge has launched AID-SURG, an AI-based program for tailored perioperative care in CRC surgery. The program includes interventions before, during, and after surgery, as well as after discharge, to support faster recovery. AID-SURG is supported by AIS-1, a locally operated Clinical Decision Support System (CDSS). AIS-1 provides a one-year mortality prediction to help CRC surgeons choose the best treatment options and improve survival for patients undergoing planned, curative cancer surgery. In this master’s thesis, we explore surgeons’ experiences of working with AIS-1 using ethnographic methods. Our review of the literature highlights a lack of research on what it means to integrate CDSSs into clinical practice. Based on our findings, we conclude that further training and clear information are needed for successful adoption, and that surgeons’ attitudes and visions for AIS-1 influence whether the system is sustained in their practice.

Tyk- og endetarmskræft (colorectal cancer, CRC) er en alvorlig global sygdom, der årligt forårsager omkring 935.000 dødsfald. For at imødegå dette har Kirurgisk Afdeling på Sjællands Universitetshospital, Køge, etableret AID-SURG, et AI-baseret program for skræddersyet perioperativ pleje i forbindelse med CRC-kirurgi. Programmet omfatter indsatser før, under og efter operation samt efter udskrivelse for at fremme patienternes restitution. AID-SURG understøttes af AIS-1, et klinisk beslutningsstøttesystem (CDSS), som drives og anvendes lokalt på afdelingen. Formålet med AIS-1 er at hjælpe CRC-kirurger ved at give en ét-års mortalitetsforudsigelse, der kan støtte valg af den bedste behandling og øge overlevelsen for patienter, der gennemgår planlagt, kurativ kræftkirurgi. I dette speciale undersøger vi kirurgernes erfaringer med at bruge AIS-1 ved hjælp af etnografiske metoder. Vores litteraturgennemgang peger på, at der mangler forskning i, hvad integration af CDSS betyder i en klinisk kontekst. På baggrund af vores indsigter finder vi, at yderligere træning og klar information er nødvendig for en vellykket anvendelse i klinisk praksis, og at kirurgernes holdninger og visioner for AIS-1 påvirker systemets fortsatte brug i deres praksis.
