Abstract
This study investigates the use and perceived usability of the MineAftaler app among pregnant women with diabetes receiving care at Steno Diabetes Center North Denmark. The app enables secure, written two-way communication between pregnant women and healthcare professionals regarding blood glucose management, treatment adjustments, and pregnancy-related concerns, and also provides an overview of upcoming appointments and digital appointment management. In a quantitative usability evaluation, two data sources were combined: (1) message data from 100 pregnant or previously pregnant women with gestational diabetes mellitus (GDM), type 1 diabetes (T1D), or type 2 diabetes (T2D), and (2) questionnaire data from 15 current or former users. Messages sent through the app were analyzed descriptively by diabetes type, gestational week, and predefined themes, and differences in messages per active week per participant were assessed using non-parametric statistical tests. A total of 1189 patient messages were included, and participants sent on average 11.89±16.41 messages during the observation period. Messages per active week per participant differed significantly between diabetes types (p=0.03), with higher activity among women with T1D compared with those with GDM (p=0.01). Mean message activity per week per participant was 1.06±0.91 before gestational week 20 and 0.75±0.85 after, remaining below the local recommendation of at least two contacts per week after week 20. The most frequent message themes concerned blood glucose and treatment regulation. Questionnaire responses, mainly from women with GDM, showed a mean System Usability Scale (SUS) score of 78.17±17.15, indicating generally good perceived usability, and most respondents reported that the app improved their overview of the treatment pathway, although some technical issues were noted. Overall, the findings suggest that MineAftaler is a useful digital communication tool in pregnancy diabetes care, but that engagement and communication patterns vary between patient groups, highlighting the need to align implementation with clinical context and group-specific needs.
