Danmark har i en årrække prioriteret digitalisering af sundhedsvæsenet, men indførelsen af nye nationale standarder har ofte givet problemer, fordi fokus har ligget på modulopbygning og teknisk integration frem for sameksistens med eksisterende it-systemer. Dette speciale undersøger, hvordan man kan sikre sameksistens, når en ny standard indføres, og udvikler en generel metode til at identificere problemområder. Metoden afprøves i en dansk case om Fælles Sprog III (FSIII) og Fælles Medicinkort (FMK) gennem struktureret litteratursøgning, kvalitative interviews og observationer i hjemmesygeplejen i Aalborg Kommune samt informations- og arbejdsgangsanalyser, herunder gennemgang af medicinmodulet i EOJ-systemet Care. På baggrund af analysen formuleres konkrete designforslag, blandt andet om genbrug af data fra FMK, sammenhæng mellem plejeplan og FSIII samt håndtering af ydelsesfrekvens og opfølgning, og der udarbejdes guidelines til at forebygge sameksistensproblemer. I casen viste metoden sig at kunne identificere væsentlige problemområder og uhensigtsmæssige arbejdsgange og at danne grundlag for at analysere udfordringer ved at udvikle design og løsninger. Arbejdet peger på, at en systematisk tilgang kan støtte implementeringsbeslutninger ved indførsel af nye sundheds-it standarder og har potentiel relevans ud over den konkrete case.
Denmark has prioritized e-health for years, yet introducing new national standards has often proved difficult because emphasis on modularity and technical integration has overshadowed coexistence with established systems. This thesis examines how to ensure coexistence when a new standard is introduced and develops a general method to identify problematic areas. The method is evaluated in a Danish case involving Common Language III (FSIII) and the Shared Medication Record (FMK) using a structured literature search, qualitative interviews and observations in Aalborg Municipality home nursing, and information and workflow analyses, including a review of the medication module in the Care electronic record system. Based on the analysis, the thesis proposes concrete design solutions—such as reuse of FMK data, alignment between care plans and FSIII, and handling of service frequency and follow-up—and formulates guidelines to prevent coexistence problems. In the case, the method identified key problem areas and inefficient workflows and supported analysis of challenges in designing solutions. The work indicates that a systematic approach can inform implementation decisions when introducing new health IT standards and may be relevant beyond the specific case.
