The project derives from two of my prior educational courses; first, my bachelor project from the studies of social work, which was an exploring examination on the effort of aftercare as a social study. The project resulted in the development of a help-to-self-help app for young people in aftercare.
Additionally, the project builds upon my internship at the Mental Health Services, Capital Region of Denmark, Secretariat and Communications during my master’s degree in social work. One of my main assignments was to help develop a working tool for network meetings – meetings between the vulnerable citizen and relevant professional experts.
In this thesis, I have used my knowledge and experience from especially those two courses, partly my knowledge on the segment of vulnerable children and partly my experiences with improvement projects.
The project contains a short presentation of the sociopolitical and legal conditions through the years.
The thesis uses perspectives on segment estimation concerning aftercare, it applies the perspective on segment estimation in the matter of aftercare and is based on interviews with four informants from four different municipalities.
The objective of the project was to gain insight into the specific criteria on which it is decided whether a vulnerable young person receives the social effort of aftercare or not, in preparation for how it is possible to qualify the segment estimation with especially focus on the young person’s involvement.
The project is based upon a hermeneutic theory of science and an action-oriented research strategy. It applies theories on experiences of coherence, appreciatory and solution-focused approaches and organizational theories to conduct an analysis of the statements from the four informants around the following themes: Segment estimation and involvements, intersectoral collaboration and knowledge sharing, and improvement measurements.
My intention was to design a pilot or draft working tool to be used in the municipalities to ensure consistent, systemic, well-coordinated casework and for the purpose of supporting the young people’s experiences of coherence based on an appreciatory and solution-focused approach.
The draft working tool is presented at the end of the thesis and is based on my knowledge from the analysis. Finally, my thesis contains a conclusion about other expressed improvement measurments.
