Dette speciale tager udgangspunkt i den stigende sociale ulighed i sundhed i Danmark og undersøger, hvordan en lungemedicinsk afdeling i Region Nordjylland kan styrke sin kommunikative organisatoriske sundhedskompetence (KOS) for at støtte patienter med kronisk obstruktiv lungesygdom (KOL). Formålet er at identificere og foreslå konkrete ændringer, der kan gøre det lettere for alle patienter – uanset sundhedskompetenceniveau – at forstå, bruge og handle på sundhedsinformation. Projektet er planlagt med MRC-frameworket og anvender INDEX-guidance som udviklingskøreplan. Datagrundlaget omfatter en spørgeskemaundersøgelse (med få respondenter), en systematisk litteraturgennemgang og kvalitative interviews. Det lave svarantal begrænser muligheden for at drage sikre konklusioner om, hvordan patienter og personale oplever den nuværende kommunikation, eller om der er forskelle mellem grupperne. På baggrund af litteraturen og interviewene udarbejdes fem forandringsforslag: differentieret konsultationstid tilpasset patientens sundhedskompetence, fælles konsultation med læge og sygeplejerske, indførelse af understøttende kommunikationsstrategier, systematisk feedback på de ansattes kommunikation samt specialespecifik undervisning af personalet. Forslagene kan samlet set understøtte en styrket KOS på afdelingen og dermed bidrage til at reducere social ulighed blandt patienter med KOL. Samtidig påpeges, at KOS kun er ét element af organisatorisk sundhedskompetence, og at der er behov for yderligere undersøgelser af andre elementer og evaluering af de foreslåede tiltag.

This thesis addresses the growing social inequality in health in Denmark and examines how a respiratory department in the North Denmark Region can strengthen its communicative organizational health literacy (KOS) to better support patients with chronic obstructive pulmonary disease (COPD). The aim is to identify and propose practical changes that help all patients—regardless of health literacy level—understand, use, and act on health information. The project was planned using the MRC framework and guided by INDEX for the development process. Data sources included a questionnaire survey (with few respondents), a systematic literature review, and qualitative interviews. The small sample limits firm conclusions about how patients and staff currently perceive communication or whether their perceptions differ. Drawing on the literature and interviews, the project proposes five changes: allocating consultation time differentiated by patients’ health literacy, joint consultations with a physician and a nurse, introducing supportive communication strategies, systematic feedback on staff communication, and specialty-specific training for staff. Together, these proposals may strengthen communicative organizational health literacy in the department and help reduce social inequality among COPD patients. The thesis also notes that KOS is only one component of organizational health literacy, underscoring the need to examine other elements and evaluate the proposed initiatives.
