Abstract
Internationalt er det en kendt udfordring at dele information på tværs af sundhedssektorer. Dette projekt undersøger, hvordan information i dag flyder mellem primær sektor (Nærverkið, hjemmeplejen) og sekundær sektor (Landssygehuset) på Færøerne, og hvordan et e‑sundhedssystem kan understøtte processen. Casestudiet kombinerer interview med ledere og sygeplejersker med en journal-audit, dvs. en systematisk gennemgang af patientjournaler og relevante skabeloner/blanketter. Som teoretisk ramme anvendes Jens Müllers teknologibegreb. Analysen viser uklarheder i både den dokumentation, der deles, og i selve udskrivelsesforløbet. Der er uoverensstemmelser mellem sektorernes beskrivelser, og begge parter udtrykker behov for mere fokus på formidling for at sikre sammenhængende patientforløb. Arbejdsgangene ved udskrivelse er uændrede efter indførelsen af hospitalets e‑sundhedssystem, og karakteren af den delte dokumentation er heller ikke ændret. Overgangen mellem sektorerne rummer en “gråzone”, hvor ansvarsfordelingen er uklar. I lyset af den færøske digitaliseringsstrategi om et samarbejdende sundhedsvæsen med “én hukommelse”, anbefales det at implementere e‑sundhedssystemet i primær sektor med særligt fokus på gråzonen og med inddragelse af fagpersoner fra begge sektorer i implementeringen.
Sharing information across parts of the healthcare system is a well-known international challenge. This project examines how information currently flows between the primary sector (Nærverkið, home care) and the secondary sector (the National Hospital) in the Faroe Islands, and how an e‑health system could support this process. The case study combines interviews with managers and nurses with a journal audit, that is, a systematic review of patient records and relevant templates/forms. Jens Müller’s concept of technology provides the theoretical lens. The analysis shows ambiguities in both the documentation that is shared and in the discharge process itself. Accounts from the two sectors are inconsistent, and both express a need to focus more on communication to ensure coherent patient pathways. Discharge workflows have not changed since the hospital introduced its e‑health system, and the nature of the documentation exchanged between sectors has also remained the same. The transition between sectors contains a “grey zone” where responsibilities are unclear. In line with the Faroese digitalization strategy—aiming for a collaborative healthcare service with “one memory”—the project recommends implementing the e‑health system in the primary sector, with special attention to the grey zone and with professionals from both sectors involved in the implementation.
