Specialet undersøger, hvordan klinikere på otte sygehusafdelinger i Region Midtjylland kan understøttes i implementeringen af Den Digitale Forløbsguide, en digital app til patientinformation. Som et kvalitativt casestudie inspireret af en hermeneutisk tilgang indsamles og analyseres data løbende gennem desk research af eksisterende evalueringer, interviews med implementeringskonsulenter og klinikere, deltagerobservationer ved implementerings- og evalueringsmøder, et litteraturstudie samt et fokusgruppeinterview. Analysen inddrager forandringsledelsesteori, herunder Kotters 8-trinsmodel og Six Sources of Influence, for at koble fund til mulige indsatser. Studiet identificerer seks overordnede faktorer, der påvirker implementeringen: tid til at arbejde med Forløbsguiden, arbejdsgruppens sammensætning, patientforløbenes karakter, appens design og tekniske funktioner, målsætning og evaluering af resultater samt forankring af nye arbejdsgange. På den baggrund formuleres anbefalinger rettet mod regionsledelsen, implementeringskonsulenter og de kliniske afdelinger, herunder tydeligere målsætning og opfølgning, vedligehold og forankring af forandringen, udbygning af konsulenternes rolle og involvering, anerkendelse af klinikernes kompetencer samt styrket vidensdeling og netværk. Specialets iterative design bidrager med en nuanceret forståelse af klinikerdrevet implementering og konkrete forslag til at understøtte processen.
This thesis examines how clinicians in eight hospital departments in the Central Denmark Region can be supported in implementing Den Digitale Forløbsguide, a digital app for patient information. Using a qualitative case study inspired by a hermeneutic approach, data were collected and analyzed iteratively through desk research on existing evaluations, interviews with implementation consultants and clinicians, participant observations at implementation and evaluation meetings, a literature review, and a focus group interview. The analysis draws on change management frameworks, including Kotter’s 8-step model and the Six Sources of Influence, to link findings to potential interventions. The study identifies six overarching factors that shape implementation: time to work with the app, composition of the project group, the nature of patient pathways, the app’s design and technical functions, goal setting and evaluation of results, and the anchoring of new workflows. Based on these, the thesis proposes recommendations for regional management, implementation consultants, and clinical teams, including clearer goal setting and follow-up, sustaining and anchoring the change, expanding the consultants’ role and involvement, recognizing clinicians’ competencies, and strengthening knowledge sharing and networks. The iterative design provides a nuanced understanding of clinician-led implementation and practical suggestions to support the process.
