Specialet adresserer den folkesundhedsmæssige udfordring med social ulighed i sundhed og behovet for mere målrettede anvisninger til, hvordan kommunalt ansatte sundhedsprofessionelle kan inddrage borgeres lokale netværk, særligt for borgere i sårbare positioner. Formålet er at undersøge, hvordan sundhedsprogrammer kan designes, så lokale netværk bliver aktivt involveret, og hvordan sundhedsprofessionelle kan styrke denne inddragelse for at øge social støtte. Metodisk kombineres en systematisk litteratursøgning, der afdækker centrale designelementer i sundhedsprogrammer, med en samskabelsesproces bestående af workshops med 15–25 sundhedsprofessionelle på et kommunalt sundhedscenter; analysen er informeret af Berkman et al.’s teori om sociale relationers betydning for helbred. Litteraturen peger på, at brug af fysiske faciliteter i nærmiljøet, gruppeformat, samarbejde på tværs af organisationer og serviceudveksling i lokalsamfundet kan facilitere social støtte. På den baggrund samskabes en prioriteret liste over seks væsentlige elementer til at styrke inddragelsen af lokale netværk i overgangen fra kommunale sundhedsforløb: identificering af borgerens motivation, sundhedsprofessionelles kendskab til tilbud, tværfaglig koordinering, samarbejde med brobygger, relationel støtte gennem identificering af støttende faktorer i borgerens netværk samt en trinvis og langsom overgang. Specialet bidrager med kontekstspecifikke anbefalinger til praksis og peger på behovet for yderligere samskabende processer med flere relevante interessenter for at imødegå mulige utilsigtede konsekvenser.

This thesis addresses the public health challenge of social inequality in health and the need for more targeted guidance on how municipal health professionals can involve citizens’ local networks, especially for those in vulnerable positions. It aims to explore how health programs can be designed to actively include local networks and how professionals can enhance this involvement to strengthen social support. The study combines a systematic literature review, identifying key design elements of health programs, with a co-production process involving workshops with 15–25 health professionals at a municipal health center; the analysis is informed by Berkman et al.’s theory on the health impact of social relationships. The literature indicates that using physical facilities in the local environment, group-based formats, collaboration across organizations, and community service exchange can facilitate social support. Building on this, the project co-produces a prioritized list of six essential elements to strengthen local network involvement during transitions from municipal health programs: identifying the citizen’s motivation, professionals’ knowledge of available services, interdisciplinary coordination, collaboration with a bridge builder, relational support through identifying supportive factors in the citizen’s network, and a gradual, slow transition. The thesis offers context-specific recommendations for practice and calls for further co-production with additional stakeholders to address potential unintended consequences.
