Specialet undersøger, hvordan brobyggersygeplejersker ved Aalborg Universitetshospital i Region Nordjylland trækker grænser i deres arbejdsliv, når de støtter mennesker i udsatte situationer med multisygdom og komplekse behandlingsforløb. Funktionen blev etableret i 2019 for at mindske ulighed i sundhed ved at koordinere på tværs af sektorer og specialer. Med afsæt i en konstruktivistisk og interaktionistisk tilgang og inspireret af orienterende begreber om grænsearbejde, integration og segmentering mellem arbejde og privatliv, copingstrategier samt frontstage og backstage, gennemførte vi kvalitative short-term shadowing-observationer af tre ud af fem deltagende brobyggersygeplejersker og semistrukturerede interviews med alle fem. Analysen har to spor. For det første viser den, at brobyggersygeplejerskerne har en høj grad af selvtilrettelæggelse og et bredt handlerum, som kræver, at de aktivt afgrænser opgaver og målgruppe; deres grænsearbejde er situationsafhængigt, forhandles med samarbejdspartnere og afspejles i brugen af arbejdstelefon og -computer, som enten skærmer eller sammenblander arbejde og privatliv. For det andet viser den, at relationsarbejdet med patienter kræver en kombination af nærvær og professionel distance, understøttet af kollegiale fællesskaber og backstage-praksisser som faglig sparring, ventilering og sort humor. Specialet bidrager med nuanceret indsigt i en central funktion for mere lige adgang til sundhed og peger på behovet for fortsat forskning i, hvordan disse professionelle kan støttes.
This thesis examines how Nurse Case Managers at Aalborg University Hospital in the North Denmark Region draw boundaries in their work when supporting vulnerable people with multimorbidity and complex treatment trajectories. The NCM function was introduced in 2019 to reduce health inequality by coordinating across sectors and medical specialties. Using a constructivist and interactionist perspective and drawing on orienting concepts of boundary work, integration and segmentation of home and work, coping strategies, and frontstage/backstage, we conducted qualitative short-term shadowing of three of the five participating NCMs and semi-structured interviews with all five. The analysis has two strands. First, NCMs operate with a high degree of self-organisation and a broad space for action, which requires them to actively delimit tasks and target groups; their boundary work is situational, negotiated with colleagues and partners, and reflected in the use of work phones and computers that either separate or blend work and home. Second, effective relationship work with patients hinges on combining personal presence with professional distance, supported by communities of coping in backstage spaces that allow professional discussions, feedback, venting, and gallows humour. The thesis offers a nuanced constructivist-interactionist account of NCMs’ boundary practices in support of more equal access to care and underscores the need for continued research on how to sustain and support this role.
