Abstract
Background: Evidence suggests that individuals with anorexia nervosa (AN) have a higher than average intelligence quotient (IQ) compared to the IQ levels of standard-ized norms. This hypothesis of the intellectual functioning of individuals with AN can lead to serious consequences in clinical practice, if clinicians attribute patients with AN a higher than average IQ, than what they in reality have. Therefore, the aim of this master’s thesis is to investigate what the full-scale intelligence quotient (FSIQ) among clinical patients with anorexia nervosa is and how it is distributed, by system-atically appraising the available research evidence. Methods: To accomplish this, a systematic review was conducted. To ensure that the reporting was of high quality the PRISMA 2020 guidelines were used. Research evi-dence was secured through a systematic search on March 18th, 2024 in three electron-ic databases: Embase, PsycInfo and PubMed. Peer-reviewed primary literature pub-lished in English, Danish, Norwegian or Swedish that reported FSIQ measures of clinically diagnosed patients with AN was included. Additionally, only studies that derived the FSIQ from either the Wechsler Intelligence Scales or the National Adult Reading Test were included. No date, age or sex restrictions were made. 900 articles were identified and screened with a priori set inclusion and exclusion criteria, and the quality of the studies was assessed using the Critical Appraisal Checklist for Co-hort Studies and the Critical Appraisal Checklist for Analytical Cross-sectional Stud-ies. Results: The screening resulted in 40 articles eligible for qualitative data synthesis, of which 32 were cross-sectional studies and 8 were cohort studies. The total number of study participants for this systematic review is 2148. The results of the synthesis show, that clinical patients with AN have FSIQ-scores in the average distribution area which ranges from 90-109. Discussion: Most of the included studies were of medium quality as assessed by the quality assessment checklists. Some limitations of the studies include sparse reporting of methodology and possible sampling-bias. Because of this the conclusions derived from these studies should be interpreted with caution. Conclusions: Even though the results from this systematic review show, that patients with AN have average IQ levels, contrary to former evidence, which found that the IQ is higher among these individuals, it is recommended, that clinicians still assess the intellectual functioning of their patients with AN, to be sure, not to over- or un-derestimate the IQ levels of the patient.
